Surgery for Dissociated Vertical Deviation: For Transposition of the Inferior Oblique Muscle

There are now a large number of reports in the literature documenting the effectiveness of the inferior oblique (IO) anterior transpositioning (AT) procedure for DVD, particularly for DVD's under 20PD. Patients with congenital esotropia who develop inferior oblique overaction, are treated with an anterior transposition when recessing the inferior obliques to correct both the inferior oblique overaction and to prevent or correct DVD. This has greatly reduced the incidence of superior rectus muscle (SR) surgery for DVD. When patients have DVD without inferior oblique overaction, the AT procedure is usually not recommended, rather SR recession or IR resection is preferred.
